The Community Re-Entry Network Program is a program of the Houston Health Department. Our caring, multi-disciplinary staff is committed to addressing the needs of the “whole” person with innovative, evidence-based programming and linkage to services designed to meet the needs of formerly incarcerated persons, and interrupting the cycle of recidivism. 

The Diabetes Awareness and Wellness Network (DAWN) Center provides free diabetes self-management education and many other health related services to Houstonians in order to reduce and prevent chronic disease.

The Harris County Area Agency on Aging is part of a nationwide network of agencies coordinating supporting services for adults 60 years or older and their caregivers.
